Updated fillable trust forms available at AFT website

Alabama Family Trust is pleased to work with attorneys as you help clients establish special needs trusts. Toward that end, we have updated all the forms you need to create a first-party, third-party revocable or third-party irrevocable trust. 

The forms are available on our website under the Documents tab here. We also have a checklist to assist you in making sure you have everything necessary to create the trust. 

For your convenience, and as requested by attorneys, the forms are in fillable PDF format as appropriate. 

We advise individuals who contact us that we do not dispense legal advice, and we encourage them to contact an attorney to ensure that the trust complies with rules and regulations of Social Security and Medicaid. 

Similarly, Alabama Family Trust does not provide legal advice to attorneys regarding whether you should use a supplemental needs trust in a particular situation; however, we will assist you in establishing the trust for your client. We are willing to appear in court to answer any questions a judge may have regarding the trust and the way the trust operates at the time of establishing the trust.
